Kidney Disease & Dialysis Support
Chronic kidney disease and dialysis-dependent renal failure place extraordinary demands on the body and require daily clinical management that most home care models are not equipped to provide. Without skilled oversight at home, the gaps between dialysis sessions become windows of serious risk. Legacy Concierge provides RN-directed kidney disease management that closes those gaps, keeps clients stable between treatments, and ensures the full clinical picture is always visible to the care team.

Our nurses provide the precise daily monitoring and clinical coordination that renal disease demands — managing fluid balance, protecting dialysis access, and keeping clients stable through every phase of treatment.
Fluid Intake, Output & Weight Monitoring
Consistent tracking of fluid balance and daily weight between dialysis sessions — the most reliable early indicators of fluid overload and cardiovascular strain in renal failure patients.
Dialysis Access Site Assessment & Infection Prevention
Skilled evaluation of fistulas, grafts, and catheters for infection, clotting, and structural changes — protecting the access that dialysis-dependent patients rely on entirely for their treatment.
Medication Reconciliation & Renal Dosing Oversight
Comprehensive medication review with careful attention to renal dosing requirements — managing the significantly elevated toxicity risk that impaired renal clearance creates across multiple drug classes.
Post-Dialysis Monitoring & Recovery Support
Clinical assessment following dialysis sessions for fatigue, hypotension, and recovery status — with direct escalation when post-treatment findings indicate unusual physiological demand.

Understanding the Condition
Kidney disease is a systemic condition with no tolerance for gaps in management. These are the clinical realities that make consistent daily oversight essential between dialysis sessions and beyond.

What our nurses watch for
The period between dialysis sessions is when clinical risk is highest. Our nurses monitor for signs of fluid overload including unexpected weight gain, increasing shortness of breath, and edema in the extremities. We watch for electrolyte imbalance indicators including muscle weakness, cardiac rhythm changes, and altered mental status. Dialysis access sites are assessed regularly for infection, clotting, or structural changes that require immediate attention. Every finding is communicated directly to the nephrology team. Families shouldn't have to wonder whether something is normal. That's what we're here for.
