Founded in 2022, Legacy Concierge was built to address what the healthcare system consistently fails to provide: integrated, continuous oversight of the whole person. Our model is led by a Registered Nurse who holds clinical accountability for the individual — unifying nursing, professional caregiving, and care coordination under a single framework.
Legacy Concierge addresses what the traditional model cannot — by combining clinical leadership, hands-on care, and seamless coordination into a single accountable framework.
Every client is supported by a dedicated Registered Nurse — their RN Ambassador — who oversees the care team, monitors health, coordinates with physicians, and evolves the care plan as needed.
From professional caregiving and daily living support to complex skilled nursing and specialty clinical care — all within one coordinated model. As needs evolve, the team evolves. No transitions. No gaps in continuity.
The RN Ambassador communicates clinician-to-clinician with physicians, keeps families informed, and coordinates every part of the care plan. Families step out of the complexity — because that complexity now belongs to us.

These four principles are not displayed on a wall. They are embedded in every hire, every care plan, every clinical decision, and every interaction Legacy Concierge has with the families and partners who trust us.

Jason Kidushim founded Legacy Concierge with a commitment to redefine what private care should deliver — not only clinically, but ethically. The standard he set was not informed by market research. It was informed by loss.
Witnessing his grandfather's battle with Alzheimer's disease exposed what happens when a family navigates a complex medical system without structured support — the confusion, the fragmentation, the erosion of dignity that occurs when no one is truly in charge of the whole picture. That experience became the founding conviction of Legacy: that this should never have to happen to another family.
Drawing on decades of leadership in healthcare operations, patient advocacy, and service design, Jason established Legacy Concierge as a nurse-led private care organization grounded in disciplined standards and genuine human connection.
Under his leadership, Legacy has grown into a clinically rigorous model designed for families navigating high-acuity care, cognitive decline, chronic illness, and life transitions — with the discretion, continuity, and precision the traditional system cannot match.
Because a family's legacy is not just a story — it is a responsibility. Every care plan, every clinical hire, and every partnership is selected with intention, ensuring client success.
