Licensing Disclosure
Legacy Concierge is licensed by the California Department of Public Health (CDPH) as a Home Health Agency. Our license number is 550009533. Licensing information may be verified through the CDPH Healthcare Facility Licensing & Certification Program.
Home Care Aide Registry
All home care aides are registered with the California Home Care Aide Registry maintained by the California Department of Social Services (CDSS), as required by Health & Safety Code §1796.12. Clients may verify aide registration at: https://www.cdss.ca.gov
Background Check Requirements
All caregivers undergo criminal background checks through California DOJ LiveScan fingerprinting and, where applicable, FBI background checks, as required under Health & Safety Code §1796.17.
Client Bill of Rights
As a client of Legacy Concierge, you have the right to:
- Be treated with dignity, respect, and courtesy.
- Receive services without discrimination based on any protected characteristic.
- Be informed of your plan of care and participate in care decisions.
- Privacy of your personal and health information under HIPAA and the California CMIA.
- File a complaint without fear of retaliation.
- Receive a written copy of this Bill of Rights.
- Know the qualifications and license status of your caregivers.
- Be free from abuse, neglect, and exploitation.
Mandatory Reporting
Under California Welfare & Institutions Code §15630 and Penal Code §11166, Legacy Concierge staff are mandated reporters required to report known or suspected abuse or neglect of dependent adults, elders (65+), and children to Adult Protective Services (APS) and Child Protective Services (CPS).
