Anxiety & Depression
When anxiety or depression occurs alongside serious medical illness, the clinical picture becomes significantly more complex. Mental health conditions affect medication adherence, appetite, sleep, motivation to engage with care, and the body's ability to heal. Legacy Concierge provides an integrated approach that treats the whole person — ensuring the psychological dimensions of care receive the same clinical attention as the medical ones.

Our nurses integrate psychological monitoring into every clinical assessment — managing psychiatric medications, coordinating with behavioral health providers, and treating mental health as inseparable from physical recovery.
Psychological Symptom Monitoring
Consistent assessment of mood, behavioral patterns, and psychological status integrated into every clinical visit — with early identification of deterioration before it affects adherence, engagement, or physical health outcomes.
Psychiatric Medication Management & Interaction Oversight
Careful oversight of psychiatric medications in the context of complex medical regimens — managing the interaction risks that arise when mood-stabilizing agents are combined with cardiac, neurological, and other drug classes.
Behavioral Health Coordination
Direct communication with psychiatrists, psychologists, and behavioral health providers — ensuring the psychological care plan is aligned with the medical one and that no dimension of the patient's health is managed in isolation.
Sleep, Social Engagement & Family Support
Clinical management of sleep disturbance given its bidirectional relationship with mood, alongside isolation prevention planning, family education, and caregiver direction for consistent emotional support.

Understanding the Condition
Mental health and physical health are not separate clinical domains — they are deeply interconnected. These are the realities that make integrated psychological oversight essential in medically complex care.

What our nurses watch for
In medically complex patients, psychological deterioration is easy to miss or misattribute. Our nurses monitor for declining medication adherence, withdrawal from daily activities and social engagement, changes in appetite and sleep that signal worsening mood, cognitive symptoms that may indicate depression rather than neurological decline, and expressions of hopelessness or despair that require immediate clinical attention. We maintain close communication with the behavioral health team and treat the psychological dimension of care as inseparable from the physical one. Families shouldn't have to wonder whether something is normal. That's what we're here for.
