Post-ICU & Critical Illness Recovery
Surviving a critical illness is only the beginning. Patients discharged from the ICU often return home with profound physical weakness, cognitive impairment, psychological trauma, and a medical profile that bears little resemblance to who they were before admission. Legacy Concierge provides RN-directed post-ICU recovery care that treats this transition with the clinical seriousness it deserves — supporting survivors through one of the most demanding recoveries in medicine.

Our nurses provide the comprehensive clinical oversight and coordinated rehabilitation support that post-ICU recovery demands — addressing physical, cognitive, and psychological recovery simultaneously from the moment of discharge.
Discharge Assessment & Recovery Baseline
Comprehensive clinical evaluation at discharge to establish individualized recovery baselines across physical, cognitive, and psychological domains — the foundation for every clinical decision that follows.
Medication Reconciliation & Complex Regimen Management
Complete reconciliation of complex post-ICU medication regimens with ongoing oversight — managing the substantial pharmacological burden that critical illness survivors carry home from the hospital.
Respiratory Monitoring & Ventilator Weaning Support
Consistent assessment of respiratory status with ventilator weaning support where indicated — addressing the ongoing pulmonary vulnerability that follows mechanical ventilation and critical respiratory illness.
Rehabilitation Facilitation & Psychological Support Coordination
Facilitation of in-home physical, occupational, and speech therapy alongside coordinated support for PTSD, depression, and anxiety — conditions that affect a significant proportion of ICU survivors.

Understanding the Condition
Post-ICU recovery is one of the most complex and underaddressed transitions in medicine. These are the clinical realities that make structured, multi-system oversight essential from the first day home.

What our nurses watch for
Post-ICU recovery is non-linear and full of clinical variables. Our nurses monitor for signs of infection or sepsis recurrence, respiratory deterioration in patients who required mechanical ventilation, cognitive regression that may indicate an underlying medical cause, nutritional decline that is impeding physical recovery, and psychological distress that is affecting the patient's ability to engage with rehabilitation. We maintain direct communication with the discharging ICU team and all follow-up specialists throughout the recovery process. Families shouldn't have to wonder whether something is normal. That's what we're here for.
