Multiple Sclerosis (MS) Care
Multiple sclerosis is a chronic, unpredictable neurological condition that presents differently in every person and can change without warning. Relapses can introduce new deficits overnight, while gradual progression quietly erodes function over time. Legacy Concierge provides RN-directed MS care that keeps clients stable, safe, and as independent as possible through every phase of the condition.

Our nurses provide the consistent monitoring and proactive coordination that MS demands — tracking symptom patterns, managing medications, and responding quickly when something shifts.
Symptom Monitoring & Neurological Assessment
Consistent tracking of fatigue, spasticity, sensory changes, and neurological fluctuation against an individualized baseline — with direct escalation to the neurology team when new or worsening symptoms emerge.
Medication & Disease-Modifying Therapy Management
Oversight of disease-modifying therapies and MS-related medications, with ongoing monitoring for therapeutic efficacy and side effects and direct coordination with the prescribing neurologist.
Relapse Identification & Rapid Escalation
Early identification of relapse indicators and immediate communication with the neurology team — ensuring the window for intervention is never missed and permanent deficit risk is minimized.
Rehabilitation Facilitation & Mobility Support
Coordination of in-home physical therapy alongside structured caregiver direction for mobility assistance, transfers, and daily safety planning to preserve function and independence.

Understanding the Condition
MS is defined by its variability — no two presentations are the same, and no two days are guaranteed to be either. These are the clinical realities that make consistent daily oversight essential.

What our nurses watch for
MS relapses and progression can be subtle in their earliest presentation. Our nurses monitor for new weakness or sensory changes, increasing spasticity, bladder dysfunction, cognitive shifts, and fatigue patterns that deviate from the client's established baseline. We track medication response over time and communicate directly with the neurologist when symptoms suggest a relapse is underway or the current treatment plan requires reassessment. Families shouldn't have to wonder whether something is normal. That's what we're here for.
