Wound Care Management
Wounds that are improperly managed at home can escalate quickly — infection, tissue breakdown, and delayed healing are among the most common and preventable causes of hospital readmission. Effective wound care requires more than daily dressing changes. It requires skilled clinical assessment, an understanding of how wounds progress through each stage of healing, and the ability to recognize when something has changed. Legacy Concierge provides RN-directed wound management delivered consistently, in the home, with the clinical precision of an acute care setting.

Our nurses provide the consistent clinical oversight and hands-on wound management that healing requires — monitoring progression, preventing infection, and coordinating directly with the treating team at every stage.
Serial Wound Assessment
Comprehensive evaluation of tissue integrity, perfusion, and drainage patterns at every visit — with direct provider communication when wound status raises clinical concern.
Dressing Changes & Infection Prevention
Dressing changes performed to clinical protocol with strict aseptic technique, alongside active monitoring for early infection indicators before they have the opportunity to escalate.
Wound Progression Documentation
Detailed documentation of wound status over time, providing the treating physician and wound care specialist with a clear clinical picture to guide ongoing treatment decisions.
Specialist & Provider Coordination
Direct communication with wound care specialists, surgeons, and primary care physicians — ensuring clinical findings are acted on promptly and the care plan remains current.

What our nurses watch for
Wound complications rarely develop all at once — they build gradually through signs that require a trained clinical eye to catch. Our nurses monitor for changes in drainage color, volume, and odor, signs of surrounding tissue breakdown, increasing warmth or erythema, and any indication that healing has stalled or reversed. When something shifts, we act immediately and communicate directly with the treating provider. Families shouldn't have to wonder whether something is normal. That's what we're here for.
